W-9 / FEIN Request
For legal reasons, W-9/FEIN requests must be submitted for legal review. Clubs are NOT allowed to fill out W-9 forms, or give out FEIN numbers.
How to submit the form:

Download and complete the attached W-9 FEIN Request Form, and email it to 1099report@wal-mart.com
