Our Mission: Empowering everyone on their path to better health.
About us
Neighborhood Family Practice was founded in 1980 in response to a concern that residents of Cleveland’s near west side lacked access to primary health care. A Federally Qualified Health Center (FQHC), NFP focuses on providing high quality primary care in a medical home setting. As a health care provider, our seven convenient locations offer same day appointments. The NFP service area spans twelve neighborhoods across Cleveland and Lakewood and is comprised of a very diverse mix of residents. NFP accepts Medicaid, Medicare, most commercial insurance plans and offers a financial assistance program (sliding fee scale) for the uninsured. Neighborhood Family Practice is a grantee of the Health Resources and Services Administration (HRSA) and each of its locations are covered by the Federal Tort Claims Act (FTCA).

Roya Berrhouma’s journey with Aleksandra “Sasha” Rabinovich, APRN-CNP at our W. 117th Community Health Center is more than just a medical story—it’s a tale of hope, resilience, and the power of compassionate care. Roya, along with her family, embarked on a new chapter in the United States five years ago, leaving behind their homeland of Tunisia. However, amidst the challenges of adapting to a new life, Roya found herself battling hair loss and crippling fatigue. “We all trust her,” Roya smiles with gratitude as she recounts the transformation that unfolded within her family. Her daughter Rosie found relief from headaches and a troublesome rash, while her husband Naoufel, was finally able to quit smoking. Now, as Roya and her family prepare to transition to NFP’s new W. 130th Community Health Center, their hearts brim with anticipation. It’s not just about the expansion of services; it’s about the profound sense of belonging they’ve found. “The staff at NFP work with love,” Roya reflects, her words echoing the deep sense of connection and compassion that defines her family’s experience. In Sasha, Roya and her family have found more than just a healthcare provider—they’ve found a home, a sanctuary of healing, and a beacon of hope for the future. Financial support is needed for the construction and infrastructure required to convert the retail space into a medical office space. NFP has secured $1.98 million to date. With the new space being listed unexpectedly and our need to quickly move forward to secure the site, philanthropic support is essential to help us close the funding gap. Will you help? A donation of any amount will close the gap. Your investment will also connect more individuals and families to health care services now and into the future. Neighborhood Family Practice reposted this
I am honored to announce the arrival of my first child, Vera Imani, born on May 18. This new bundle of joy has brought immense light into our lives. Her birth was beautifully assisted by NFP’s very own midwifery team. Before my wife and I became pregnant, we heard countless patients share their wonderful NFP midwifery birth stories. We knew this was the experience we wanted for our family. As a health care leader, I believe every family deserves this kind of care and support. That’s why I am asking for your support for Neighborhood Family Practice’s annual fundraiser. This year, the event falls on my birthday, August 22, and your sponsorship will go a long way in helping families find their medical home. Dr. Prakash Ganesh wears many hats, including professor. Last week, he taught students from the Master of Public Health program at Case Western Reserve University's School of Medicine at our Ridge Health Center. Lauren Lasko, another NFP provider, is also a student in his class. #nfpmedcenter #clevelandcommunity #casewesternreserveuniversity

Today is World Refugee Day and we deeply value our refugee population here at NFP. We recently got to sit down with Azahar AloBadi who originally went to Turkey from Iraq. From Turkey, she came to the United States and has been here for over 10 years. She has also been a patient at NFP for over 10 years and said she has found a second home. Equitable healthcare access is critical in ensuring every refugee receives the care and support they deserve. Together, we can heal and empower communities worldwide. Last week, NFP had the pleasure of hosting KeyBank employees for their day of service! They transformed the garden in the back of our Tremont location, and we are so excited to witness the vibrant transformation ahead! Thank you, KeyBank!
