Position:

Manages a team of Registered Nurses and Medical Coordinators who oversee the medical care of individuals with intellectual and development disabilities. Works collaboratively with physicians, interdisciplinary teams, individuals in services, providers, and families to promote positive outcomes. Applies clinical knowledge to assess individuals and provide consultation and training. Assesses plans, implements, coordinates, monitors, and evaluates services required to support the individual’s health needs. Actively participates in strategic plan development and implementation.

Duties & Responsibilities:

  • Develops and updates Policies and procedures in accordance with current standards of practice using evidence-based practice guidelines and ensures regulatory compliance.
  • Develops and approves methods of writing and updating health services treatment guidelines, Care Plans, Protocols, polices and procedures for residential and nursing staff to follow when caring for individuals in services and ensure correct implementation regulatory and best practice compliance to ensure compliance with home and community-based services.
  • Ensures compliance of applicable rules and regulations, best practice related to assessments, nursing documentation, transcription of orders and other processes and systems assigned to nursing
    • Analysis completed though audits, direct audits, observations, and management of electronic health records.
  • Monitors medical support services to assure individuals in services needs are being met in a timely and efficient manner.
  • Directs the supervision of implementation and documentation of nursing process that include assessment, nursing diagnosis and developing, directing and implementing care plans and the evaluation of the effectiveness of the plan in accordance with current standards for practice.
  • Completes and documents evaluations and assessments to identify training and support needs to ensure continuity of care.
  • Develop strategic plan that incorporates data analysis to improve processes organizational to promote better patient outcomes with a focus on health and safety medical
  • Provides medical consultation to interdisciplinary team members both internally and externally.
  • Interdepartmental collaboration and weekly meetings
  • Participate in agency regulatory/monitoring committees: such as but not limited to hospital review subcommittee and Quality Assurance Performance Improvement etc., provide input and assistance in ensuring quality care and best practice guidelines are met
  • Attending external stakeholder training, meetings, and nursing conferences applicable to patient population.
  • On-call rotation approximately every 10 weeks with rotating holidays

Other Duties as Assigned

  • Depending on the needs of the nursing department there may be situation where the Associate Director of Nursing may have to assist in day-to-day obligations such as delegation, QMAP and MRB training, day to day nursing functional duties as well as day to day medical coordinator responsibilities and other duties.

Supervisory Responsibilities:

  • Manages agency wide nursing professionals and medical coordinators through direct supervision.
  • Carries out supervisory responsibilities in accordance with organization’s policies and procedures and current nursing practice guidelines using evidenced based guidelines.
  • Interview, hire, and train employees; plan, assign and direct work; performance appraisals; reward and correct/discipline action; address complaints and resolve problems
  • Manages by reviewing performance measures in accordance with agency’s mission, efficiency, effectiveness regarding optimal services and fiscal responsibility. Ensure department is following guidelines for best practice under the regulatory requirements to obtain the highest quality of care.
  • Must have high integrity, act as a role model for direct reports, and have strong management/leadership skills

Minimum Qualifications & License/ Certification Requirements:

  • Bachelor’s degree in nursing from an accredited institution
  • Current, valid licensure as a Registered Nurse from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC)
  • Two (2) years professional healthcare experience

Preferred Qualifications:

  • Experience as a Registered Nurse working with individuals with intellectual and developmental disabilities and mental health diagnosis.
  • Knowledge and experience of physical conditions and treatment protocols for conditions of the IDD population.
  • Experience multi-tasking and prioritizing assignments
  • Nurse case management experience
  • Three years of employee supervisory experience
  • Caring, supportive, accountable leader
  • Leader willing to work on an interdisciplinary team and problem-solve solutions to improve services

Conditions of Employment:

  • Must possess a valid unrestricted U.S. Drivers License
  • Travel: This position must travel to remote work sites and utilize personal vehicle. Submit mileage reimbursement.
  • Must pass a Colorado Bureau of Investigation and a Federal Bureau of Investigations background check as a condition of employment

Location: This position is primarily a work from home position. This role requires occasional travel to our offices (Aurora, Loveland, Colorado Springs, Pueblo) and the homes of the individual’s we serve (across the front range).

Physical Demands:

While performing the duties of this job, the employee is regularly required to reach with hands and arms, squat, go up and down stairs, talk or hear. The employee frequently is required to drive. The employee must occasionally lift and/or move up to 50 pounds. This job's specific vision abilities include close vision, distance vision, color vision, depth perception, and ability to adjust focus

Job Type: Full time

Pay: $65,000 - $85,000

Schedule: Monday- Thursday. This position offers a flexible work schedule that may vary based on need. This position is not required to work weekends or carry an on-call phone.
