Job Summary

The Clinical Data Specialist, under the supervision of the Director of Clinical Operations and in collaboration with the Medical Director and the Director of Quality and Compliance, is responsible for assisting with developing and implementing the annual QAPI plan, including quality initiatives and projects. This position has the responsibility to provide comprehensive support for quality Participant care, including assisting in determining what quality measures to evaluate, ensuring the staff has the needed training and tools to improve the quality of care, and facilitating discussions, feedback and follow up to ensure sustained improvements. Under the general direction of the Director of Clinical Operations, the function of the position is to ensure systematic review and analysis of the clinical practices, identifying potential and actual adverse events, as well as assisting with follow up on employee/Participant incidents.

Essential Duties And Responsibilities

  • The Clinical Data Specialist maintains a working knowledge of applicable Federal, State, and local laws and regulations, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.
  • Supports personnel from other departments through coaching and mentoring to help achieve optimum standard of excellence. Addresses issues of concern through courageous conversation and notifies department managers of any interactions requiring attention.
  • Under the supervision of the Director of Clinical Operations and in collaboration with the Medical Director and the Director of Quality and Compliance will assist with, initiating, developing, and guiding the implementation of the annual Quality Assurance Performance Improvement Plan.
  • Ensure compliance with CMS and DHCF regulations
  • The Clinical Data Specialist assists with the data collection, report preparation and review of the quality and effectiveness of patient care provided by Clinical and Medical personnel
  • Assists the Director of Clinical Operations, Medical Director, and Director of Quality and Compliance to prepare for CMS site visits, inspections, and all subsequent follow up activities
  • Assists the Director of Clinical Operations, Director of Quality and Compliance and Medical Director to plan, develop, organize, implement, evaluate, coordinate, and direct the infection control program in accordance with current rules, regulations, and guidelines that govern such requirements, including conducting an annual review of the program for approval by the PACE Quality Management Committee
  • Assist to prepare monthly summaries of all participant/personnel infections, hospitalization, ER visits and other quality measures, corrective action taken, and the results of the corrective action as assigned
  • Ensure accurate and timely reporting to CMS, State, and all other PACE reporting requirements.
  • Assist staff, Director of Quality and Compliance and Medical Director in establishing process evaluation, including root cause analyses and performance improvement initiatives
  • Maintain confidentiality of participant information
  • Attend and participate in staff meetings, in-services, other projects, and committees as assigned

Qualifications

Education

  • Must possess a comprehensive knowledge of healthcare delivery, compliance and management at a level as normally obtained through an Bachelor's Degree in Nursing practice, Public Health or other healthcare related fields and approximately two (2) years or more of progressively more responsible related work experience that includes the development and implementation of programs for the elderly population, service operations, and staff development. A combination of education and experience may be acceptable.

Experience

  • Minimum of two (2) years of documented experience working with a frail or elderly population preferred
  • Minimum of two (2) years of demonstrated successful experience in Home Health Care
  • Minimum of three (3) years of documented experience working in the field of compliance for a healthcare organization
  • Ability to research, analyze and assimilate information from various on-site or virtual sources based on technical and experience based knowledge. Must exhibit critical thinking skills and possess the ability to prioritize workload.
  • Must submit to and pass a pre-employment drug/alcohol screening and criminal background check.
  • Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records.
  • Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public.
  • Must be free of communicable diseases that can be readily transmitted in the workplace. TB test is required within the first 7 days of employment. Hep B vaccinations are offered but not required.
