PurposeCare and its affiliate companies are looking for a qualified, dependable, honest, reliable, Intake Nurse (RN/LPN) to serve as an Intake Nurse Coordinator to support those caring for the needs of our clients. The Intake Nurse provides a broad range of administrative functions such as receives, evaluates, and coordinates referral activities. Verifies third-party reimbursement coverage, as needed. Distributes information to appropriate division for scheduling of employees.
Essential Job Functions
Receives and processes all requests for services.
Establishes system for prior authorization from third-party payers.
Collaborates with Business Development Staff in providing education and resources to Agency staff
and community.
Demonstrates teamwork and effective communication to accomplish Agency and patient goals.
Performs job in compliance with Agency policies and procedures and professional and community
Provides services in accordance with state and federal laws, and ACHC standards.
Performs other duties and activities as deemed necessary.

Qualifications And Requirements
Current license with the state Board of Nursing (RN/LPN)
1 year of nursing experience required
At least one-year experience of home care intake referral experience, preferred
Knowledge of community resources, Medicare and state regulations, ACHC requirements, and reimbursement criteria for third party and private payers.
Excellent communication skills (verbal and written), interpersonal skills, and documentation skills.
Ability to successfully complete a background check, professional reference check, and drug/alcohol screening test
