This position provides services to those living in the Agency’s community-based houses (CBH). www.socialmodel.com
Essential Duties And Responsibilities
Employee is required to visit each house on a weekly basis, providing direct support to all occupants.
Maintain a consistent scheduled of program visits and meetings.
Communicate with the Alumni Association on any information pertaining to alumni events and updates.
Participate in the Continuing Care Connection project.
Responsible for all CBH policies and procedures.
Sustain maintenance plans for all houses.
Maintain a safe environment for all occupants, including training on First Aid and Narcan delivery.
Responsible for keeping census of each house at capacity.
Monitor internal and external cleanliness of each house.
Maintain strong working relationships with designated liaisons in each Agency outpatient program to ensure continuity of treatment relationships to housing.
Responsible for monthly and fiscal reporting for department’s census, budget, and on-going development.
Familiarity and adherence to all regulatory expectations and ethical standards and Agency policies and procedures, including the Agency’s Core Beliefs.
Maintain constructive relationships with family members, community members and organizations, human services agencies, and other professionals.
