What Will You Be Doing
The Intake Coordinator coordinates activities of order intake, evaluation of admission criteria, referral/authorization of cases, and transition of care for the Home Health Agency, tracking of referrals.
What Will You Come With
Minimum of two (2) year of healthcare experience, in home healthcare preferred.
Specialized knowledge
Knowledge of the Home Health Services
Knowledge of referral/authorization process
Knowledge of computer clinical programs such EHR/EMR system(s)
Skills & Abilities
Great communication skills, both written and spoken.
Bi-lingual preferred
Strong organization skills and be detail oriented.
Ability to maintain confidentiality of sensitive information.
