Under the direction of the VNA Manager and within established organizational and departmental policies and procedures, responsible for providing services to address the social and/or emotional factors that impact the patient’s ability to achieve the goals in the Plan of Care. Services are provided under the direction of a physician and in conjunction with other health care team members.
Qualifications

Master’s degree from a school of social work accredited by the Council on Social Work Education required. Current license as a LCSW required.
Minimum of one (1) year experience in a health care setting required, home care experience preferred.
Demonstrated strong verbal and written communication skills and good interpersonal skills required.
Demonstrated knowledge of the principles and practices of social work and of resources available in the community required.
Current driver’s license, good driving record and access to or own reliable transportation required.
