Tidewell Hospice, a member of Empath Health is seeking a Social Worker to join our team.
The Social Worker coordinates care and/or provides supportive and medical social services to terminally ill patients and their families in accordance with an appropriate plan of care. In collaboration with the interdisciplinary group, maintains responsibility for the development, implementation, and management of psycho-social services provided to hospice patients and families in all settings.
The Social Worker
Promotes and practices the Hospice mission and core values and follows all policies and procedures of Empath Hospice.
Assesses patient/family psychosocial, emotional, economic, environmental and cultural factors in order to estimate their potential to cope with terminal illness and death.
Participates in the development and revision of the plan of care (POC), including short term individual counseling, crisis intervention, and fiscal, legal and health care decisions. Involves the patient/family in the POC whenever possible. Consults and collaborates with the interdisciplinary group and others involved in the patients care.
Provides psychosocial counseling and support consistent with organization guidelines, NHPCO standards of practice and in support of patient /family’s unique spiritual and cultural beliefs.
Updates patient/family on condition and provides supportive counseling on a regular basis.
Identifies and utilizes appropriate community resources, assesses patient/family/caregiver’s ability to access them and provides education as appropriate.
Assesses need for counseling related to risk assessment for pathological grief, risk of suicide, abuse or neglect and collaborates patient/family needs with the IDG, and other community agencies as needed.
Educational/Professional
Master of Social Work (MSW) degree from a school of Social Work accredited by the Council on Social Work Education
At least one year of social work experience in a healthcare setting (the one-year Masters level internship would meet this requirement) and has a current Florida license, or
Is registered with the State of Florida as a Registered Intern actively working on licensure, or
Is in the process of becoming a Registered Intern with the State of Florida and will have a registration within 90 days of hire.
Licensure:
LCSW or registered social work intern licensed eligible.
Valid Florida State Driver’s License
For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
