Do you have difficulty treating the upper limb that has little-to-no movement? Do you find it difficult knowing where to start or how to make the therapy task-specific? If so, keep reading - and check out this video 😁 Firstly, using motor cortex priming techniques at the beginning of therapy sessions has demonstrated positive effects on motor recovery. This video illustrates a few of these techniques: 1. 🧠 Anodal brain stimulation (tDCS): typically increases neuronal excitability which can enhance cortical activity in the targeted area (in this case the effecte motor cortex) and facilitate cortical reorganisation or #neuroplasticity 2. ⚡ FES: not only has the potential to strengthen the targeted muscle group, but also increases excitability of the corticospinal tracts related to innervation of the stimulated muscles 3. 👐 Bilateral symmetrical movements: has the potential to reduce interhemispheric inhibition and induce a cross-hemisphere coupling effect (whereby the affected hemisphere can 'learn' from the unaffected) Secondly, making therapy #taskspecific or #goaldirected for the severely impaired population doesn't have to be difficult! Again, this video will show you how, but if you focus on the basic, foundational skills of reaching, grasping and releasing - you can't really go wrong! 1. 🖥 Set up the environment to support the person to engage in goal-directed activity. HINT: some assistive tech or a little extra creativity might help if the person is extremely weak 2. 🏋♀️ Design a task that can be easily adapted or graded to continually challenge the person at the 'just right level' 3. 🥅 Include a goal, an action and an outcome to all your therapy activities, this will ensure your therapy remains goal-directed and engaging, to support motor learning. In this case, the goal is to put the ball into the basket; the actions are reach, grasp, transport and release; and the outcome is success?! or do I re-set and try again? 🤠 Some context for this example: this gentleman is 8 months post stroke. He arrived at our clinic with no active movement in his arm or hand. He had been repeatedly told by professionals that he shouldn't expect any recovery in his arm. These videos were taken after just 3 hours of #evidencebasedpractice NB: all the movement you see in the second video is active, i.e. no electrical stimulation I believe in #noplateau What about you?
