How many physician office visits are there every year? Seriously - how many doctors appointments do we collectively as a population go to? According to the CDC (2019) the number of physician visits exceeded 1 Billion. 72% of those visits were attended by people 65 and over. Many of those need transportation options to get them to their appointments on time. That's where we help out in Southern California. Specifically Riverside, San Bernardino, Orange and San Diego counties. Ambulatory, Wheelchair and Gurney options. Compassion, on-time, every time, door-to-door service. 800-982-6641 for a quote. Positively moving the population health needle through Chronic Care Management one hospital at at time!
I just read a report that has me deeply concerned. It turns out that Kansas is number two on the list of states with struggling rural hospitals. As someone who cares about the well-being of our rural communities, this hits close to home. But there is hope. If you manage a rural hospital or know someone who does, let's connect. We must work together to ensure that our rural hospitals get the support they need. #RuralHealthcare #KansasHospitals #HealthcareCollaboration

Assisted living communities have incentives to reduce hospitalizations, but where to start may be a common question. Lisa Lassiter, RPh, director of clinical services for Guardian Pharmacy of South GA, suggests a crucial step is partnering and leveraging the expertise of an LTC pharmacy. Making this step a reality requires a proven community leader, like Peggy McGhee at Willow Health Care Inc, who fully understands the benefits. Read why and discover additional steps to reducing hospitalizations in this Senior Housing News article: https://loom.ly/58asTXo #LongTermCare #Pharmacy

Healthcare Narrative Strategist | Storytelling for Outcomes | Strategic Comms for Growth & Innovation
"Not only do they see a health status improvement in this population, but utilization and spending have decreased too. Cardea found that inpatient stays, ER visits, psychiatric visits, and nursing home stays have declined by nearly 80% among Oak Days residents, representing an enormous savings for the people who pay the bills — indirectly the state and now directly the health plans contracted to CalAIM. With the long-term cost of nursing home care now under managed care plans, rather than the state fee-for-service program, health plans have a vested interest in supporting people to live outside nursing homes. Thus, a model such as Oak Days could be of material interest to these plans." https://lnkd.in/g-krfYUf Great piece from California Health Care Foundation, take a look. Simply put, we’re grateful for Wisconsin hospitals and health systems and their dedication to high-quality, affordable and accessible health care: 🏆 High quality, high value care | Wisconsin has the 4th-most five-star-rated hospitals in the country. 💙 Strong safety net | Wisconsin hospitals and health systems provide care to all patients who walk through their doors, regardless of their ability to pay. Additionally, in FY 2022, Wisconsin hospitals provided over $2 billion in community benefits and charity care to their communities. 🚑 Proven commitment and reliability | Wisconsin hospitals receive more than two million emergency room visits every year, providing life-saving care to friends, neighbors and loved ones requiring urgent medical attention. Through the good times and the bad, Wisconsin hospitals answer the call. 🩺 Strong workforce | A broad distribution of hospitals in Wisconsin not only helps the state’s residents access high-quality health care, it also serves as an important local economic resource to surrounding communities. Wisconsin hospitals employ more than 110,000 people, with even more working in other care settings, like clinics. #wihospitals #grateful

Visiting is a crucial part of a person’s care and can play an important role in ensuring the safety of patients, with carers and family members often able to provide crucial insights and knowledge that can support diagnosis, care and treatment. A problem raised repeatedly during the Covid-19 pandemic, particularly in social care settings, concerned a lack of guidance and inconsistent approaches by individual organisations to implementing visiting policies (such as in this blog on the hub: https://lnkd.in/eGUwNF85). Last year the Government announced new plans to legally make visiting a part of care, and the Care Quality Commission are now consulting on its proposed guidance to help providers and other stakeholders understand and meet the new standard on visiting and accompaniment in care homes, hospitals and hospices. The consultation closes at midday on 20 February 2024 and can be found via the hub below: https://lnkd.in/ecYCkDtP #pslhub #patientsafety #socialcare #carers #adultsocialcare #carequalitycommission #familyengagement

“Starting Monday, the first of 53 Surrey Memorial Hospital patients will be moved to a new “transitional” housing facility run by Fraser Health to free up hospital beds.” Housing is healthcare. This is such an important step for our health system to be taking, recognizing the importance of providing transitional supports for people exiting hospital and ensuring continuity of care. There is so much innovation occurring in healthcare right across the province at the moment. This is wonderful to see. 👏👏👏
