📍 Congenital and Acquired Chiari Syndrome Chiari malformation, an anomaly of the posterior cranial fossa, also known as Arnold–Chiari malformation, was described in autopsies by Hans Chiari, an Austrian pathologist, in 1891. He defined four distinct anatomical configurations in which the cerebellar tonsils (the most caudal part of the cerebellum) protruded below the lower margin of the foramen magnum.1 Several subtypes have subsequently been described, often in association with other anomalies, but the essential feature remains the descent of the cerebellar tonsils below their normal position just above the rim of the posterior fossa. With the wide availability of cerebral imaging, recognition of Chiari malformations has become common, and determining which patients have symptoms associated with this anatomical abnormality can be challenging. Many patients have atypical symptoms or borderline imaging findings. For these patients, cine flow MRI can be informative.22 Research is required in order to identify patients who would benefit from surgical decompression, delineate appropriate surgical techniques, and improve our understanding of the association of CM1 with cognitive and psychiatric symptoms. let's make longevity simple by defining some key terminology: Lifespan is the number of years we live Healthspan is the percentage of lifespan lived in "good health" (physical, cognitive, emotional). Free of disease, frailty, and chronic pain. Sickspan is the percentage of lifespan characterized by frailty and disease. Our conventional medical system is very good at extending lifespan. Think cardiac bypass surgery, antibiotics, chemotherapy, other LIFE-saving technologies). Unfortunately, it's less good at preserving our health in the process. Therefore our sickspan is ever-expanding The goal of longevity medicine is 2-fold: Maximize healthspan so that it fills 99.99% of lifespan Minimize sickspan to the tiniest fraction of a percent of life When you focus on interventions that do the above, increased lifespan typically comes as a side effect Is that really such an outlandish goal?

Lessons learned from the first genetically-modified pig heart into a human patient. A new study published today in The Lancet has revealed the most extensive analysis to date on what led to the eventual heart failure in the world's first successful transplant of a genetically-modified pig heart into a human patient. This groundbreaking procedure was conducted by the University of Maryland School of Medicine (UMSOM) physician-scientists back in January 2022 and marked an important milestone for medical science. The patient, 57-year-old David Bennett, Sr., was treated at the University of Maryland Medical Center. He experienced strong cardiac function with no obvious signs of acute rejection for nearly seven weeks after the surgery. A sudden onset of heart failure led to his death two months after the transplant. Since then, the transplant team has been conducting extensive studies into the physiologic processes that led to heart failure to identify factors that can be prevented in future transplants to improve the odds of longer-term success. Today, I’m sharing my true colors for World Lymphedema Day! A day to raise awareness of lymphedema, a condition that is much more common worldwide than previously thought, is only minimally known in the medical community, and for which there are few treatment options. Fortunately, with an early diagnosis, the progression of lymphedema can be slowed down or even stabilized, and preventive work is often done during cancer operations to avoid lymphedema, and also surgery is coming up to cure secondary lymphedema in some cases. For primary (hereditary) lymphedema however, less options exist. Treatment mainly consists of compression, exercise and lymphatic drainage, every day. Which means self-management (and a close relation with the JOBST representatives 😊) The more information we have, the more early diagnoses can be made, and research that leads to therapies is sparked.
