Member Satisfaction: High satisfaction rates in long-term services and supports (LTSS) care management for six consecutive years. 4. Supporting Education: Empowering members through GED programs and education on mental health and social inclusion. 5. Healthy Rewards: Members receive rewards for maintaining good health through My Health Pays®. 6. Ten Years of Giving Back: Sunflower employees contributed about 500 volunteer hours in 2022, strengthening community ties. An inspiring story from the amazing Sian Brand to celebrate national #socialprescribingDay! Sian explains how relationships are key to partnership working and to develop community assets to improve health and wellbeing. It's hard to believe that 10 years on, we are still struggling to deliver non clinical placements in undergraduate #healthcareeducation. Without sufficient knowledge of the #socialdeterminantsofhealth through meaningful learning experiences in the #thirdsector or #socialhousing we will not achieve the changes that are needed in the way we deliver care in the #nhs Once students experience the impact of #socialprescribing or advice on prescription their practice is transformed. We need a national framework to fund and support an "alternative" placement strategy, that helps to deliver a workforce capable of achieving the #nhs long-term plan.

Happy #SocialPrescribingDay! I've been reflecting on my work in #SocialPrescribing and came across the logo of the first SP Model I facilitated & programme managed. It was in Mid Essex in 2014, pre-NHS SP, pre-Long Term Plan. It was VCFSE hosted, centred around GP Practices, co-designed with over 40 local organisations & whole systems approach. I trained over 600 SP Connector Champions to support Link Workers. A local digital platform (created & hosted by the CVS) sat at the centre public facing to connect directly with the VCSE organisations. It was truly a community wide led model, building on strengths & assets. It was so ahead of its time. My advice to others is time spent on relationship building is key. Building trust & rapport but most importantly buy in & local ownership. Also whilst the SP #LinkWorker is key, we don't want to build an alternative dependency. Your community champions (volunteers) that can help facilitate self-care, self-management & self-service are critical too. Thank you to all those involved & their bravery & faith in focusing on the community. As I ponder further the 9 years I've spent in SP I wish you all a very happy #SocialPrescribingDay! Thank you for all that you do to progress this key approach to health & wellbeing. Improving health literacy has a range of proven benefits- helping people understand the importance of looking after their own health and health information like what medication to take and how to manage their long term conditions. This is key particularly when focusing on reducing health inequalities- health information must be accessible to all.
