Nurse and other non-physician care management of COPD is shown to significantly reduce mortality and (all-cause, cardiac, and pulmonary) reduce ER and hospital utilization. Interventions are similar to pulmonary rehab and very similar to the programs provided by NuvoAir Medical. Strengths: Very large sample size with long term followup. Propensity matched cohort comparison with careful analysis of covariance. Limitations of the study: non-randomized, retrospective, may not generalize to US health care system.

Fascinating: Machine learning prompts led to a significant decrease in mortality with trend toward earlier discharge. Some highlights. 1. Careful attention was given to timing and volume of alerts , avoiding alert fatigue. 2. Some messages were sent directly to respiratory therapists, acknowledging the critical role these professionals play in managing complex patients. It's not all about the doctors, folks. 3. Machine learning is more than just dumping data through a neural network hopper.

Why does the US health care system consistently pay $100 to save $1 but resist paying $1 to save $2? At first glance, this study is great news for folks with moderate to severe COPD. 34% relative reduction in moderate/severe exacerbations per year is a big deal! However, if you look into the data (digging through online supplements) and factoring in the cost of Dupixent (Dupilumab), the cost to prevent one admission per year is $2 million. Each of these admissions should cost about $20k. NNT to prevent one admission is 35 (only 10% of the moderate to severe exacerbations were severe). Dupixent annual cost about $60k. So the ROI from a payer perspective is about -100:1. In other words, spend $100 to save $1 . That's a terrible deal. If instead we invested in supported self management programs and remote pulmonary rehab (ie NuvoAir services), we would see similar results with a positive ROI of 2:1. Spend $1 to save $2. That's a great deal.

COPD and Asthma are dramatically under diagnosed and under treated...up to 70% of people suffering these conditions are not diagnosed. These conditions are some of the leading drivers of avoidable hospital care, disability, health care spending, suffering, and death across all ages. There is absolutely a reasonable path to solving this problem if key stakeholders will join us to provide spirometry and care management in the clinic and at home for people with #asthma, #copd and other breathing problems. Congress could pass H.R.2829 - Chronic Care Management Improvement Act of 2023 and H.R.6801 - RPM Cost Sharing Elimination Study Act of 2023 which would drive affordable and high quality care for conditions like this.
