Already making an important impact, this field has the potential to transform drug development as well as the practice of medicine. Such a simple, but critically important, slide reminding us of the value of multimodal measurement. When scores from subjective and objective measures aren’t correlated, is one wrong? Or are both important? Spoiler - both are important! In the past, we’ve only had access to patient (and loved one, clinician, caregiver, etc.,) perspectives of functioning and measures of functional capacity. What the patient was actually doing day-to-day in the real-world in real-time was unknown. With the development of reliable and valid digital measures (eg., from wearable devices), we have access to passively collected objective data from the real-world providing for a more holistic assessment of the patient. While it is easy to assume these more objective measures could replace the less reliable or less accurate subjective information gathered, it is actually all of this data together that provides the greatest opportunity for personalized care and improved patient outcomes. Shelby Bachman, PhD from VivoSense, Inc. did a great job presenting a use case for the value of longitudinal multimodal measurement (holistic assessment) in oncology at the Roche #DigitalBiomarkersSummit last week in Basel.

Great to be at the Digital Biomarkers Summit at Roche in Basel. An excellent event, with great conversations, coupled with insightful presentations and panel discussions. It’s clear that cross-industry-collaboration is essential in order solve some of the most challenging problems & grateful to be part of the “tech / pharma” ecosystem. #DigitalBiomarkersSummit

One of the most exciting studies in the field of Digital Health Technologies in clinical trials was sponsored by small biotech Bellerothon. Former CEO Peter Fernandes describes how they got the go ahead from the FDA to use the amount of moderate to vigorous physical activity from a wrist wearable as a primary endpoint in their phase 3 study of an experimental treatment for lung disease. This alignment with FDA came after numerous meetings and showing data from this same endpoint in phase 2. This was exciting as it could have been the first new drug approved on the basis of a digital endpoint. But Peter Fernandes lost his job as the study was negative. And it seems the phase 2 results for their selected primary endpoint didn't translate to phase 3. Peter seems to argue amount of moderate to vigorous physical activity is just too variable a measure, especially in the context of COVID restrictions during some of their study. This gives us all cause for reflection. Peter took FDA feedback as encouragement to go ahead with a risky study design that ultimately failed. Many others in the field of digital endpoints are frustrated that the FDA much more often says "no", and is sometimes blamed for putting a break on innovation. Regulators have a difficult balance to strike: they can get criticised either way. It is really valuable that this Digital Biomarkers in Clinical Trials Summit wrestles with the challenges the field needs to navigate, as well as the opportunities.
