This position requires the individual to either be a Notary or to obtain Notary certification.

The Care Coordinator plays a crucial role in supporting the Care Manager by expanding billable hours, ensuring regulatory compliance, delivering exceptional care to seniors and disabled adults, and providing an outstanding working experience for our care team. This position requires the individual to either be a Notary or to obtain Notary certification.
Duties And Responsibilities
Client Assessments and Reassessments:
- Conduct meetings with clients and their families in their care settings.
- Obtain and document assessment information using approved procedures and forms.
- Gather relevant information from various sources, including insurance companies, financial planners, and attorneys.
- Ensure assessments are clear, accurate, and based on comprehensive data collection.
- Identify high-risk factors, barriers to goals, and strengths that will assist clients.
- Perform 90-day supervisory visits with clients.
- Develop comprehensive care plans addressing all pertinent areas of the assessment.
- Set measurable, time-specific, and attainable goals with the client and decision-makers.
- Incorporate private and community resources into care plans.
- Assist in client care coordination with outside resources, making and attending appointments as needed.
- Ensure all care plans are created and updated within 24 hours.
- Participate in quality management studies.
- Document progress and conduct quality control evaluations with clients and their families.
- Ensure timely updates to care plans based on reassessments or evolving client needs.
- Communicate all care plan modifications to caregivers and the operations team promptly.
- Conduct new caregiver orientations and ongoing training.
- Ensure caregivers are trained to company standards and maintain the DCW/PCW Program.
- Perform caregiver supervisory visits and facilitate ongoing caregiver training.
- Resolve conflicts within the care team and collaborate with the Care Manager to allocate necessary resources.
- Facilitate DCW/PCW training and oversee skills evaluations.
- Ensure the branch office adheres to local licensure laws and Medicaid regulations.
- Monitor and prepare for state survey readiness.
- Participate in quarterly PIP meetings and initiatives.
- Assist with answering company phone calls.
- Assist with preparing agendas and attend team meetings.
- Act as a positive role model when interacting with clients, employees, and the public.
- Create weekly/monthly schedules for reassessments and orientations.
- Provide backup for the Care Manager and conduct new client intake assessments as needed.
- Assist with scheduling and provide recruiting support when needed.
- Maintain client status updates and document all sales calls in Slack.
- Perform other related duties as assigned.
- Excellent customer service skills
- Skilled in caregiving, with expertise in advanced transfers such as Hoyer lifts, gait belts, and slide boards. Proficient in hands-on care, including bed baths and pressure wound prevention.
- Proficiency with computers and technology
- Strong organizational skills and attention to detail
- Team player with workplace flexibility
- Strong analytical and problem-solving skills
- Ability to function well in a high-paced and sometimes stressful environment
- Reliable vehicle and clean driving record
- At least one year of care supervision, coordination, or management.
- Ability to become DCW Trainer certified
- Prolonged periods of sitting at a desk and working on a computer
- Must be able to travel within local service area
- Must be a Notary or become a Notary
