Summary

Position Summary:

Provides supportive care to patients, residents and families. Promotes successful operations of patient/family engagement while enhancing the customer experience by providing support in the Long-Term Care and Hospital areas Essential Functions: Level of Supervision Required: Work is reviewed daily Contacts: Regular contacts, within or outside the Company to give or get information. Require courtesy, tact, and some knowledge of Company procedures. Type of Supervisory Responsiblities: None Qualifications/Requirements Entry level knowledge; basic employment skills; High School diploma: GED equivalent or High School certificate Education/Knowledge: Education Discipline: N/A Engagement and Advocacy Acts as a liaison between patients, families and the community, Long-Term Care and Hospital.

Educational Requirements

1-3 years of customer service experience in a hospital or long-term care setting
